Our doors are locked, knock on the rear door at the top of the wheelchair ramp or ring the top doorbell at the front door to enter.

All materials ordered will be held for 90 days. If materials are not picked up, THE DEPOSITED MONEY IS FORFEITED AND WE ARE NOT RESPONSIBLE FOR THE MATERIALS. We are not a storage facility.
Due to the recent changes in health care reimbursement, we now require a minimum cash deposit of $60.00 prior to services for all patients with outstanding insurance deductibles, including Medicare. You will be billed for any balance remaining or we will reimburse you any overpayment.
Also, due to reduced reimbursement, we are forced to add the following fees:
1.) Flat fee of $25.00 for the 1st copy of medical records for personal use and transfers of all other copies will be charged accordingly.
2.) Flat Fees for completion of any forms outside of office visit. Prepayment is required.
$10.00-Simple i.e., School or DMV forms.
$25.00-Complex i.e. ,Disability, third party payer forms, Military elig. Letters
3.) Fee Schedule starting at $20.00 for any extensive telephone consultations and requests for treatment over the phone (Calls exceeding 5 mins. Initiated at the request of the patient) *Please note, telephone conversation vs office visit is at the discretion of the physician.
4.) Flat fee of $50.00 for Any Missed Appointment (Reminder phone calls regarding appointments are merely made as a courtesy to you. Missed apppointment fees apply, regardless of whether you received a reminder call or not.) Missed appointment fees must be paid before scheduling any future appointments.
